Maryland Death Certificate Applications may be obtained from the Charles County Department of Health from the Annex Building information window. By appointment only on Tuesday and Thursday.

• Cash, checks or credit cards (VISA, Discover and Mastercard) accepted.
• Hours of operation for Vital Records is 8:00 a.m. to 12:00 p.m. and 1:30 p.m. to 3:30 p.m. Monday through Friday.

Current Fees
Death Certificates – $25.00 for the first copy and $20.00 each for an additional copy of the same certificate.

Death certificates may be obtained from the Department of Health for deaths that occurred in Maryland after January 1, 2015. Certificates for deaths that occurred prior to January 1, 2015 can only be obtained from the Maryland Department of Health.

All vital records may be ordered online from The Maryland Department of Health.

*Photo ID is required. If you are obtaining a birth certificate for anyone other than yourself or your child, a court order is required.
